Dear Geoffrey,

Unfortunately, SAM's report generator is not customizable.

We'd be happy to consider changing the report if there are modifications we can make that would meet your needs.

It would be possible to build your own version of SAM using the open source repositories, and access the template there -- but that requires knowledge of computer programming and learning the ins and outs of the template editor that comes with the SAM user interface development environment.
